Анотація
Мета - дослідити різнопланові підходи до лікування дегенеративно-дистрофічного ураження хребта та сформулювати найбільш ефективні методи та засоби лікування, їх етапність з урахуванням патогенетичної основи больових синдромів зумовлених цим стражданням.
Методи дослідження: бібліосемантичний, порівняльний, системний аналіз.
Результати. Різноманітність засобів лікування дегенеративно-дистрофічних уражень хребта не полегшує, а можливо, навіть ускладнює роботу практичного лікаря. Це пов’язано з тим, що інформація відносно клінічних переваг різноманітних медичних препаратів та методик, а також різного роду оперативних втручань надто суперечлива, відсутня єдина методика послідовної, комбінованої терапії больового синдрому вертеброгенного характеру. Немає ні універсального терапевтичного, ні універсального хірургічного втручання, яке б забезпечило стійке нівелювання симптомів неспецифічного болю у спині і/чи корінцевого синдрому. Головне у виборі методики лікування знаходиться у площині патогенезу розвитку конкретних клінічних проявів захворювання та у співвідношенні клінічних проявів із патоморфологічними змінами. Вирішальна роль у регресі больового синдрому належить зменшенню ступеня набряку та набухання міжхребцевого диска та корінця спинномозкового нерву, нервових закінчень. Не викликає сумнівів, що нестероїдні протизапальні препарати (НПЗП) найбільш ефективні для зняття поперекового та корінцевого болю на початку лікування. При відсутності значного поліпшення після використання НПЗП та необхідних саногенних рухових навантажень потрібно застосувати більш активні методи лікування. Це, насамперед, найрізноманітніші способи локального введення фармакологічних препаратів – від банального підшкірного обколювання болючих точок до УЗД- та МРТ- контрольованих ін’єкцій безпосередньо в ділянку навколо ушкодженого нервового корінця, в епідуральний простір, у фасетковий суглоб.
Висновки. Лише після неефективності прийому НПЗП та пункційних методів лікування, включно з проведенням епідуральної ін’єкції та у випадку стійкої механічної компресії нервових корінців необхідно переходити до відповідних хірургічних методів лікування, як малоінвазивних, так і відкритих.
