Анотація

Вступ. Тривале використання контактних лінз пов’язане з ризиком запальних захворювань поверхні ока бактеріального генезу. Важливими чинниками таких ускладнень є мікробна контамінація лінз, формування біоплівок і недостатня ефективність окремих засобів догляду. Тому актуальним є оцінювання профілактичної дії багатофункціональних розчинів щодо клінічних бактеріальних ізолятів.

Мета. Визначити спектр бактерій, які колонізують м’які силікон-гідрогелеві контактні лінзи за тривалого використання, та дослідити їхню чутливість до антибіотиків і багатофункціональних розчинів для догляду за лінзами.

Матеріали та методи. Проведено бактеріологічне дослідження нальоту з внутрішньої поверхні контактних лінз (КЛ) 60 користувачів, які не менше 3 років використовували силікон-гідрогелеві лінзи. Визначено чутливість отриманих бактеріальних ізолятів до антибіотиків, що застосовуються в офтальмологічній практиці, а також протимікробну активність двох комерційних багатофункціональних розчинів: на основі полікватерніуму і міристамідопропілдиметиламіну (№ 1) та полігексаметиленбігуаніду (№ 2).

Результати. У 46,7 % посівів виділені представники роду Staphylococcus : 16 ізолятів (26,7 % ) належали до виду S. aureus , 12 ізолятів (20 % ) – S. epidermidis . Серед інших грампозитивних мікроорганізмів у трьох випадках (5,0 % ) виділені Enterococcus faecalis , в одному випадку (1,7 % ) – Corynebacterium macginleyi . Ще у 5 % пацієнтів КЛ були колонізовані Escherichia coli . Дослідження динаміки відмирання мікробної популяції у розчинах № 1 та № 2 показало високий дезінфікуючий потенціал обох препаратів щодо стафілококів. Через 15 хвилин після внесення завису клінічного штаму стафілококів у кожен із препаратів кількість життєздатних клітин зменшувалась на lg 1,5, а через 30 хв життєздатних клітин стафілококів у розчинах не виявлялось. Культури клінічних ізолятів E. coli демонстрували найвищу здатність виживати у досліджуваних дезінфікуючих розчинах. Швидші темпи відмирання ешерихій спостерігали у розчині № 1, де кількість життєздатних клітин за 2 години зменшилась на lg 4,5. У розчині № 2 відповідний показник становив лише lg 1,5.

Виснов ки . За тривалого використання силікон-гідрогелеві контактні лінзи можуть контамінуватися бактеріальною мікрофлорою з переважанням стафілококів. Досліджені багатофункціональні розчини ефективні щодо грампозитивних коків, однак при контамінації лінз грамнегативними бактеріями, зокрема E. coli , стандартної експозиції може бути недостатньо для повного знезараження.
