Анотація
Мета. Визначити роль та місце ендотеліальної дисфункції у патогенезі коморбідного перебігу гастроезофагеальної рефлюксної хвороби (ГЕРХ) та ожиріння шляхом дослідження рівнів стабільних метаболітів оксид азота (NO) в плазмі крові та сечі. Оцінити ефективність та переваги призначеної терапії, що передбачала додаткове включення пептидної сполуки, аналогу лей-енкефаліну – тирозин-2-аланіл-гліцин-фенілаланіл-лейцил-аргініну діацетату (Даларгін).
Матеріали та методи. Проведено оцінку клінічних проявів за опитувальником GERD-HRQL, визначення рівнів стабільних метаболітів оксиду азоту NO2 та сумарних метаболітів NO2 + NO3 спектрофотометричним методом, рівня лептину імуноферментно, рН-метрію та дослідження моторної функції за УЗД. Обстежено 130 пацієнтів, з яких 70 хворих на ГЕРХ з коморбідним ожирінням І ст., 40 хворих на ГЕРХ без супутньої патології та 20 практично здорових осіб. Виділено 3 групи дослідження.
Результати. Визначено, що рівні стабільних метаболітів NO2, NO2+NO3 крові та сечі в групі ГЕРХ з ожирінням достовірно нижчі за рівні групи ізольованої ГЕРХ (p<0,001). Встановлено пряму кореляційну залежність між рівнем метаболітів оксиду азоту крові й сечі та кислотністю в шлунку та стравоході; обернену залежність між метаболітами NO та ступенем моторно-функціональних порушень ШКТ, вираженістю клінічних проявів, рівнем гормональної секреції адипоцитами. Додактове включення Даларгіну дало змогу покращити ефективність та якість патогенетичної терапії та досягти більш позитивної динаміки захворювання та рекомендовано до призначень у практиці.
Висновки. Ожиріння є предиктором більш тяжкого перебігу ГЕРХ та вираженішої ендотеліальної дисфункції. Існують кореляційні зв’язки між вираженістю ендотеліальної дисфункції та ступенем функціональних, моторних та секреторних порушень шлунка та стравоходу.
