Abstract

Introduction. Currently, motion-preserving surgical techniques have gained particular attention in scoliosis correction, especially Anterior Scoliosis Correction (ASC) and Vertebral Body Tethering (VBT). Despite the growing interest, the number of studies evaluating the effectiveness of ASC/VBT remains limited, and the assessment of postural balance following these procedures is still insufficiently explored.

Aim. To assess the dynamics of postural balance restoration in patients with idiopathic scoliosis with Lenke 5C and 3BN curve types following ASC/VBT.

Materials and methods. The study included 13 patients (age 17.3 ± 6.2 years) with thoracolumbar deformities classified as Lenke 5C and 3BN who underwent anterior dynamic scoliosis correction (ASC/VBT). Postural balance was evaluated using the “Ergo Therapy” software. Radiographic assessment was performed at three time points: preoperatively, 2–3 weeks postoperatively, and 6 months after surgery. Statistical analysis was conducted using SPSS 26.0.

Results. Clinical observation demonstrated that postural balance restoration after ASC/VBT in patients with Lenke 5C and 3BN deformities occurs progressively. At 3 weeks postoperatively, pronounced postural asymmetry persisted, particularly shoulder asymmetry (3.25 ± 1.62°), scapular asymmetry (4.15 ± 1.86°), and waist triangle asymmetry (3.69 ± 1.58°). At 3 months, these parameters showed statistically significant improvement (reduced to 1.47 ± 0.94°, 2.06 ± 1.36°, and 1.0 ± 0.38°, respectively; p < 0.05), indicating active restoration of static and dynamic trunk equilibrium. At 6 months, the achieved improvements remained stable, reflecting consolidation of postural balance and the formation of symmetry in the shoulder girdle, scapular region, and waist contour. The most pronounced improvement (80–90%) was observed in scapular alignment and waist triangles, corresponding to normalization of trunk muscular tone.

Conclusions. Postural balance after ASC/VBT recovers progressively: the most significant improvement occurs within the first two months, while from 2 to 6 months the parameters stabilize, indicating completion of major stages of muscular adaptation.
