Анотація
Мета. Проаналізувати характеристику нозокоміальних інтракраніальних інфекційних ускладнень (НІІУ) у пацієнтів з тяжкими вогнепальними пораненнями головного мозку (ВПГМ) у проміжному післяопераційному періоді (ППП) та розглянути раціональні підходи до антибіотикотерапії (АБТ).
Матеріали та методи. У проспективному дослідженні для порівняльного аналізу пацієнтів з НІІУ тяжких ВПГМ у ППП та оцінювання раціональних підходів до АБТ було відібрано 11 пацієнтів з НІІУ і сформовано контрольну групу з 21 пацієнтів без інфекційних ускладнень. Відбір біоматеріалу у пацієнтів, таксономічну ідентифікацію та визначення чутливості виділених культур до АБТ проводили уніфікованими методами за чинною нормативно-методичною документацією МОЗ України, Рекомендаціями Європейського комітету з тестування чутливості до антимікробних препаратів (EUCAST). Критичний рівень достовірності p < 0,05.
Результати. За даними аналізу відношення шансів, інфекційні ускладнення були фактором ризику , пов’язаним з ~17-разовим підвищенням шансів летального результату (95% ДІ 2,8–102,1). ROC-аналіз продемонстрував, що комбінована модель з шістьма предикторами (≤ 8 балів за Шкалою ком Глазго, наявність фрагментів кісток, внутрішньошлуночковий крововилив, перелом основи черепа, витік спинномозкової рідини (СМР) з рани та трансвентрикулярне ураження), має високу дискримінаційну здатність (AUC 059; 0,959–1,000). У більшості випадків діагностовано менінгоенцефаліт (100%) та вентрикуліт (27,3%). Померло 5 пацієнтів. Летальність за внутрішньочерепної інфекції, етіологічно пов’язаної з грамнегативною флорою, склала 45,4%. За інфекції, викликаної грамнегативними бактеріями групи ESKAPEE, зі СМР було ізольовано Klebsiella pneumoniae — у 7 (63,6% летальність 42,8%), Pseudomonas aeruginosa — у 3 (27,3% летальність 66,7%), Acinetobacter baumannii — у 1 (9,1%). Показано полірезистентність до карбапенемів у 4 випадках: 3 з K. Pneumoniae (42,8%) та 1 з P. Aeruginosa (33,3%), у 1 випадку — до колістину у K. Pneumoniae (14,3%).
Висновки. За тяжких ВПГМ у ППП у пацієнтів з НІІУ домінує грамнегативна мікрофлора (особливо K . pneumoniae ), яка відрізняється полірезистентністю до АБТ та супроводжується високою летальністю до 45,4%. У зв’язку зі стійкістю до АБТ та труднощами з проникненням у СМР мають бути розглянуті високодозова АБТ, пролонгована інфузія та інтратекальне/внутрішньошлуночкове введення.
