Анотація
Вступ. Нещодавні дослідження остаточно підтверджують загальновизнану думку, що нейтрофільні дерматози (НД), які виникають у поєднанні з мієлоїдними злоякісними новоутвореннями, головним чином мієлодиспластичним синдромом (МДС) та гострим мієлоїдним лейкозом (ГМЛ), не є рідкістю та часто пов'язані з поганим прогнозом.
Мета. Вивчити ефективність інтеграції нутрітивного пробіотичного компоненту в схему комплексного менеджменту хворих на МДС які мають НД.
Матеріали та методи. З 72 хворих на МДС, які перебували під клінічним спостереженням, у 34 (47,2%) хворих спостерігалися прояви лімфоцитарної інфільтрації шкіри у формі НД. Як нутритивний ад’ювант для зменшення системного запального фону цим хворим було рекомендовано вживати дієтичну добавку – пробіотик, що містить 5 видів Bacillus впродовж 12 тижнів. У ретроспективну групу порівняння були включені хворі на МДС, які отримували лікування за аналогічною схемою і не приймали нутрітивних ад’ювантів з пробіотиками. До та після лікування у хворих оцінювалася наявність клінічних проявів НД а також проводився скринінг цитокінового профілю.
Результати. У групі дослідження зафіксовано більш значуще зниження рівнів IL-6 та TNF-α, що свідчить про здатність пробіотичного засобу моделювати системний запальний процес у такий спосіб, щоб обумовлювати зниження сироваткових маркерів запалення. Після проведення нутритивної пробіотичної корекції в досліджуваній групі ми спостерігали більш акцентований регрес шкірних симптомів НД у порівнянні з ретрогрупою контролю, що може бути підтвердженням опосередкованого впливу зменшення інтенсивності запального процесу на шкірні ураження. В групі дослідження варто відмітити скорочення частки хворих з такими проявами НД, як папули, на 37,5% та пухирі – практично вдвічі. При цьому в групі контролю кількість хворих, що мали пухирі, також зменшилася у два рази, а частка пацієнтів з папулами – зменшилася на 18,8%, p˂0,05.
Висновки. Отримані результати дозволяють розглядати пробіотичні засоби як нутритивні ад’юванти, що можуть діяти з залученням вісі «кишківник-шкіра» і чинити системну протизапальну дію впродовж менеджменту хворих на МДС, які мають клінічні прояви НД.
