Анотація
Вступ. Технологічні трансформації в охороні здоров'я – цифровізація, системи штучного інтелекту (ШІ), телемедицина, генетичне редагування – відкривають нові клінічні можливості та водночас формують простір новітніх біоетичних ризиків. Питання про те, де проходить межа між технологічною доцільністю та людськими правами, залишається відкритим у світовому дискурсі.
Мета. Проаналізувати вплив сучасних технологічних трансформацій на дотримання фундаментальних принципів автономії, людської гідності, інтегральності та справедливості в клінічній медицині; обґрунтувати необхідність впровадження людиноцентричної моделі та підходу ethics by design у процесі розробки та імплементації медичних інновацій.
Матеріали та методи. Виконано наративний огляд: проведено всебічний пошук у міжнародних наукометричних базах даних Scopus, PubMed, Web of Science за ключовими термінами за період 2014-2024 рр. До аналізу було додатково залучено чинні нормативні та декларативні документи ВООЗ, ЮНЕСКО, Ради Європи та Європейської Комісії. Використано методи системного, біоетичного, компаративного, деконструктивного аналізу та порівняльно-правового аналізу.
Результати. Встановлено, що феномен алгоритмічного патерналізму та непрозорість систем ШІ («чорні скриньки») формалізують процедуру отримання інформованої згоди, підриваючи автономію пацієнта. Деперсоналізація через «датафікацію» особи й редукцію людини до цифрового профілю або датасету створює прямі загрози людській гідності, перетворюючи клінічні дані на комерційні активи. Генетичні (CRISPR-Cas9) та нейротехнології актуалізують принцип інтегральності – особливо в контексті розмивання меж між терапією та антропологічним покращенням (human enhancement). Цифровий розрив та структурна алгоритмічна упередженість (algorithmic bias) підривають принцип справедливості, обмежуючи доступ до високотехнологічної допомоги вразливим групам.
Висновки. Подальший розвиток клінічної медицини потребує обов'язкової імплементації концепції ethics by design - інтеграції біоетичних принципів у весь девелоперський та технологічний цикл інновацій. Це має супроводжуватися жорсткими правовими механізмами забезпечення прозорості алгоритмів, збереженням принципу людського контролю (human-in-the-loop) та адаптацією національного законодавства до міжнародних етичних стандартів.
