Анотація
Мета: провести аналіз і узагальнення наукових публікацій щодо залучення пацієнтів до спільного ухвалення рішень та партнерської участі в процесі лікування.
Матеріали і методи. Проведено аналіз та узагальнення наукових публікацій щодо запровадження стратегії спільного ухвалення рішень та залучення пацієнтів до самоконтролю стану здоров’я, перебігу захворювань та участі в процесі лікування. Використовувались методи: системний підхід, бібліосемантичний, аналітичний.
Результати. В сучасних умовах партнерська участь пацієнтів в процесі лікування стала обов’язковою ланкою розвитку пацієнт-орієнтованої моделі медичної допомоги і рекомендована ВООЗ. Необхідною складовою цього процесу є впровадження стратегії спільного ухвалення рішень лікарем і пацієнтом. Організаційні підходи щодо впровадження стратегії спільного ухвалення рішень в клінічну практику удосконалюються і узагальнюються, зокрема, в 2021 р. експертами NICE (Велика Британія) опубліковано настанову NG197 «Спільне ухвалення рішень». Одним із найважливіших факторів впровадження методу спільного ухвалення рішень у клінічну практику є навчання медичних працівників щодо удосконалення комунікаційних навичок та емпатії, залучення пацієнта до самостійного контролю захворювання, тощо. ВООЗ приділяє значну увагу залученню пацієнтів до самоконтролю стану здоров’я та результатів лікування, як складової партнерської участі пацієнтів в організації медичної допомоги. Доведено, що програми самоконтролю зменшують кількість незапланованих госпіталізацій хворих на хронічне обструктивне захворювання легень та бронхіальну астму, серцеву недостатність тощо. Запропоновано широкий спектр підходів щодо підтримки залучення пацієнтів до самоконтролю стану здоров’я та партнерської участі в процесі лікування, зокрема, інформаційні листівки, онлайн-підтримка рівних, індивідуальні консультації, групові навчальні заняття, коучинг по телефону, технології для моніторингу симптомів і втручання щодо зміни психологічної поведінки.
Висновки. Впровадження пацієнт-орієнтованої моделі медичної допомоги потребує залучення пацієнтів до процесу лікування, як партнерів, що передбачає участь у спільному ухваленні рішень лікарем і пацієнтом та самоконтроль стану здоров’я. Важливе значення має покращення комунікативних навичок лікарів та навчання пацієнтів щодо участі в спільному ухваленні рішень та самоконтролі.
